In some cases, it is important for your child to have reports that demonstrate the progress they have achieved or the plans you may have for their upcoming therapy and care. NAPA Centre has a range of reports that are able to be requested to assist you with this need.
To request a report from NAPA you must complete this Report Request Form.
NAPA Centre NDIS reports are written to provide all the required information that your NDIS planner will be looking for when considering your child’s capacity building and all equipment needs over the next 12 months. Whether you are preparing for your child’s first plan, their annual plan renewal, an unscheduled plan review, or a full plan appeal, we work with you to ensure that the report covers the history of your child, their current goals, progress they have made, skills they are working towards, and their overall future goals.
Please carefully consider the goals and needs you are putting forward as part of your child’s plan, as they will be required from you in the report request. Our best results with NDIS reports are where you have fully briefed NAPA so we can best cover all their needs. Please allow some time to complete your report request form and discuss with your child’s team prior to starting the form so you are aware of the full scope of their therapy plan and any assistive technology recommendations.
NDIS now requires all Low-Cost, Mid-Cost and High-Cost Assistive Technology requirements to be itemised in your child’s annual NDIS report for consideration in their plan. If you require Assistive Technology in the middle of your plan that has not been considered in the planning, it may trigger a review of your child’s plan. Please note, these reports are complex and can take some time to prepare. Be aware of your child’s annual plan dates and meetings and provide us ideally at least 45-days to prepare. NAPA cannot guarantee we can meet a NDIS planning meeting date if requests are supplied with 45 days of your meeting date.
Part of every NAPA intensive program is an Intensive Summary Report (ISR). This is automatically included and will appear as a seperate line item on your child’s Intensive invoice. This report will provide the specific goals set by the therapists, the progress that was made against their goals, any other achievements taht were made during the intensive which were outside the goals set, and the home exercise program that we provided to families. This report will be emailed to you within 3-6 weeks after the completion of your intensive.
Whilst the Intensive Summary Report contains useful information about your child’s progress during that specific intensive, it does not cover your child’s annual planning needs required for your planning meetings. We recommend a full NDIS Report is completed for all NDIS Reviews and annual planning meetings.
Assistive technology is an umbrella term that includes assistive, adaptive, and rehabilitative devices for people with disabilities, while also including the process used in selecting, locating, and using them. NDIS require a specific scripting report for all equipment items over the value of $15,001. Most of these items will also require trials of different brands to ensure the most appropriate fit and function for your child.
Upon request, an Assistive Technology Report can be written to support you in attaining funding for equipment or technology valued at over $15,001 needed to support the development of your child. Trials must have been completed and the specfic item chosen prior to submission of this report request. Most equipment in this category is bespoke for your child and may have complex additional supports. Examples of these items are:
As part of the report fee, NAPA will liaise with suppliers to arrange quotes and if required, equipment trials. Please note however that appointments for equipment trials must be booked separately and is not included in the Report fee.
Please note that NAPA is unable to comment on equipment we have not seen your child use or trial. NAPA is also unable to report on equipment for vehicles, bathrooms, home modifications and certain complex wheelchair needs, but we can refer you to some other providers.

NDIS also requires High-Cost Assistive Technology requirements to be itemised in your child’s annual NDIS report for consideration in their plan. If you require Assistive Technology in the middle of your plan that has not been considered in the planning, it may trigger a review of your child’s plan.

NDIS require a Letter of Support (NDIS refers to this as ‘written evidence’) for all equipment items between the value of $1,500 – $15,000 that have been recommended by your therapist for daily use in relation to your child’s disability. At the time of your planning meeting, you do not need to have completed equipment trials or know the specific make and model of the item you need. Once the funds are in your child’s plan for this item, you can complete equipment trials to ensure the most appropriate fit and function for your child, and then order the specific item you require. If the equipment trial was completed at NAPA, we will provide you with the required documentation with recommendations for the specific piece of equipment and attachments (NDIS calls this “written advice”).

The letter of support is not as comprehensive as a full Assistive Technology Report, which may be required for more complex items valued over $15,000. If you are not sure what you need to support your request, it is recommended that you discuss the item with your NDIS LAC. The NDIS has recently updated the way that all Assistive Technology is now reviewed and reported on and is now split into three categories. Each require different levels of reporting by your allied health therapy team.
Assistive Technology (AT) is an umbrella term that includes all assistive, adaptive, and rehabilitative devices for people with disabilities, while also including the process used in selecting, locating, and using them. Consider any equipment or individual adaptive support or device that your child needs to help with all daily living tasks, learning, positioning, communicating, or getting around – from a wheelchair through to a pair of supportive shoes or special spoon, as Assistive technology.
Planning for your child’s Assistive Technology / Equipment needs over the next 12 months is very important, and all items must now be included in your child’s annual reporting to be considered in their annual NDIS plan development and funding.
If any new Assistive Technology is required within a NDIS plan date, the whole plan may need to be submitted for a review, which could affect your child’s overall funding, and prolong the decision process. So, discussing with your child’s therapy team and planning in advance is highly recommended.
Low-Cost Assistive Technology can often be bought “off-the-shelf” and from non-disability suppliers. Examples are:
Mid-Cost Assistive Technology – any item valued between $1,501 – $15,000. These items are more bespoke for your child’s needs and may need to be ordered from specific disability suppliers. Funding for these items is drawn from the Capital Budget in your child’s plan.

High-Cost Assistive Technology – any item valued over $15,001. These items are bespoke and may have additional supports for your child’s individual complex needs. All items will need to be trialled, scripted, and ordered from specific disability suppliers. Funding for these items is drawn from the Capital Budget in your child’s plan.

Different reports take differing amounts of time to complete and need to be allocated into our therapists’ schedules well in advance to complete them.
In general, most reports will take around 45-days to be completed, especially if there are multiple therapists who are contributing. Where possible, please be mindful of requesting any report needs well in advance of any meeting dates.
If your report is urgent and you require it earlier than our standard 45-day turnaround, then you will need to pay a report rush fee for the service. Please note that this is strictly subject to availability on the therapists’ schedule, and we may not always be able to accommodate. In general, report fees can be paid for using your NDIS Funding, however this may be dependent on your child’s individual plan and how your funding is currently allocated. Please note, reports for school and legal reports may not be covered by NDIS funding in certain circumstances.
We suggest that you take the time to complete the Report Request form thoroughly so that all information you would like included in the report is represented how you would like it.
Once your report is finalised and sent to you, it cannot be changed in any way as it is a legal document. We will gladly correct any mistakes or omissions on our part, but if changes are requested based on new information or a partially completed Report Request form, then a report amendment fee will be charged.
The information contained in all NAPA Reports contain privileged or confidential information, including patient information protected by federal and state privacy laws. It is intended only for the use by the person(s) intended.
